ATHLETICS REGISTRATION ON FINAL FORMS
Fall Sports Signups are happening now, and the fall sports season is right around the corner.
START DATES:
- Football JV and Varsity: Monday, Aug. 17
- All other JV and Varsity sports: Monday, Aug. 24
- All Modified sports: Monday, Aug. 31
Plattsburgh City Schools Athletic Department is conducting registration using FinalForms to complete and sign athletic participation forms for your students.
Students cannot participate on our sports teams without first registering on Final Forms. We are no longer processing paper-based materials for athletics registration. Parents/Guardians must create an account and follow the steps. Click on the Parent Playbook (attached) to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Get Help”.
Please register at: https://plattsburghcity-ny.finalforms.com/ and follow the prompts to create your account, create your students and sign your forms.

IMPORTANT NOTES:
- All documents must be signed by parent/guardian and student prior to being cleared for their sport; this should be completed ASAP.
- If your student does not require an updated physical, dates of physicals will be updated manually by the school nurse in Final Forms.
