IF your child has signed up for a Fall Sport:
Sports start on August 18th. Practice times are To Be Determined.
For your child to be cleared for practices on August 18th, they must have:
1. An updated physical exam (within the past year)
2. An updated inhaler and/or Epipen order (if required)
3. A History Form (attached – completed, signed and returned)
If you are unable to complete the Health History Form online or print it, please pick it up (and drop off) at the PHS Main office after August 1st. The Main Office is open between 8am and 2pm, Monday through Thursday.
Please complete the Health History Form by August 11th.
If your child needs a physical exam and you are not able to get one from their provider, please contact me ASAP at csellars@plattscsd.org.
Once all documentation is received, reviewed and approved, you will receive a ParentSquare notifying you that your child is cleared.
